VOYAGE TO MARS

Scientists know a lot about Mars. Perhaps in the near future even a tripulated voyage to Mars will be possible. Scientists' telescopes have already uncovered many mysteries of Mars surface, of its athmosphere composition and they even know a lot about the interior of the planet.
We shouldn't wonder about all this; Mars distance from Earth is only, at most, 100 million kilometers after all. Much more distant galaxies, as far as thousands of millions light years away, have already been discovered.

But what about a voyage to OUR PROSTATES? No way; absolute ignorance in relative terms. Our most advanced techniques can only identify cancer in our body when there is already more than 10 million cancerous cells happily living inside us. Scientists' most advanced imaging diagnostic techniques can harldy tell whether small amounts of Prostate Cancer cells have already gone through the prostatic capsule and are invading the surrounding tissue. Our PROSTATES are still an unknown and unexplored universe.

VIAJE A MARTE

Los científicos saben mucho sobre Marte. Es incluso posible que, en un futuro próximo, haya vuelos tripulados a Marte. Nuestras técnicas más avanzadas sólo pueden detectar cáncer en nuestro cuerpo cuando ya hay más de 10 millones de células cancerosas viviendo alegremente dentro de nosotros. Las técnicas de diagnóstico por la imagen más avanzadas de nuestros científicos apenas pueden decirnos si una pequeña cantidad de células del Cáncer de Próstata ya han atravesado la cápsula prostática y están invadiendo el tejido circundante. Nuestras PROSTATAS son todavía un universo desconocido e inexplorado.

I saw the film some days ago and it made me reflect. Narnia, the land of unexpected magic reachable by chance through a wardrobe. Narnia, winter and snow forever under the power of evil, of the Ice Witch. Narnia, where the springtime, the Good, or perhaps the desire onirically represented by a Lion, fight to recover the frozen kingdom to give it back to the warmth of human senses.
I peep through the wardrobe and, under the street lamp, I can see myself staring at the dreadful castle.
I'm wondering: how did I get in here?, and nearby trees seem to whisper: -Your PCa diagnosis, can't you remember that?-
-What seemed fiction is now your reality, son of Adam, and when life pushes you through the wardrobe, your previous experiences become fiction, memories, foggy pictures-
I find some comfort thinking that, at least, the Ice Witch's Castle looks quite far away at the moment.

Mis crónicas de....Narnia

He visto la película hace algunos días, y me hizo reflexionar. Narnia, una tierra de magia inesperada a la que llegas por casualidad a través de un armario. Narnia, invierno eterno y nieve eterna bajo el poder del mal, de la Bruja del Hielo. Narnia, donde la primavera, el Bien, o quizás el deseo oníricamente representado por un león luchan para recobrar el reino helado para devolverlo a la calidez de los sentidos humanos.

Me asomo a través del armario y, bajo la farola, puedo verme a mí mismo contemplando el temido castillo.

Me pregunto: ¿cómo es que llegué aquí?, y los árboles cercanos parecen susurrar: -tu diagnóstico de CaP, ¿no lo recuerdas?-

EUNUCHS FOR HIRE

This tatoo looks original, "cool", but in fact it's more than that: it means eunuch in chinese, so no doubt that the young western man wearing this tatto will be more than surprised, if he ever finds out the translation!

Hundreds of thousands of men around the world suffering Advanced Prostate Cancer in the 21th Century, or who suffered it in the 20th Century, are not wearing this tag, but in fact they should because they are exactly that: eunuchs.

The most widely used hormonal treatment for PCa is also called, in fact, "medical castration". This is a true medical "advance", can you believe it?, because before the advent of such drugs, no more than 15 or 20 years ago, the treatment for those pacients was what is subtly termed as "orchidectomy" (cutting off your "balls" in plain English). Orchidectomy was first used to treat Prostate Cancer more than sixty years ago, and Dr. Charles Huggins received the Nobel Prize in 1966 for that discovery. Tattos were not in fashion at that time, what a shame....

How can it be that, after 39 years from that Nobel Prize, the gold standard treatment for advanced PCa is still castration, be it medical or surgical?

Ancient civilizations, as old as two thousand years BC, would be pleased to know how well, as much as unpurposedly, they were treating Prostate Cancer at that time...

SE ALQUILAN EUNUCOS

Este tatuaje parece original, "chachi", pero es de hecho mucho más que eso: significa eunuco en chino, así que no hay duda de que el joven occidental que lo lleva se quedará más que sorprendido, si alguna vez se entera de la traducción!

Cientos de miles de hombres de todo el mundo que sufren Cáncer de Próstata Avanzado en el siglo XXI, o que lo sufrieron en el siglo XX, no llevan esta etiqueta, pero de hecho deberían porque son exactamente eso: eunucos.

El tratamiento hormonal más ampliamente utilizado para el CaP se llama, de hecho, castración médica. Es un verdadero "avance" médico, ¿podeis creerlo?, porque antes de la llegada de estos medicamentos, no hace más de 15 o 20 años, el tratamiento para esos pacientes era lo que sútilmente se denominaba como "orquidectomía" (en castellano, cortarte los "huevos"). La orquidectomía se utilizó por primera vez para tratar el Cáncer de Próstata hace más de 60 años, y el Dr. Charles Huggins recibió el Premio Nobel en 1966 por ese descubrimiento. Los tatuajes no estaban de moda en aquella época, que pena....

¿Cómo es posible que, pasados 39 años desde ese Premio Nobel, el tratamiento de elección para el Cáncer de Próstata Avanzado siga siendo la castración, sea médica o quirúrgica?

Las civilizaciones antiguas, tan remotas como las de hace dos mil años AC, estarían encantadas de saber lo bien que estaban tratando el Cáncer de Próstata en aquella época sin ni siquiera propornérselo......

THE FALLEN ANGEL

If you have Prostate Cancer and you need Hormone Therapy (medical castration), you may perceive yourself like an Angel and you may feel or may fear that the people you meet and who are aware of your situation may perceive you in the same way.

Why an Angel? someone may ask: well, angels are said not to have sexual desire, and you don't have it either; further, Angels are perceived by many people as having no sexual differentiation, and you sometimes feel the same: your testicles may have shrunk, your penis may have almost disappeared and your sexual life is just history....

But, in fact, Angels are not like You by any means. You do have a sex, you are a man. Tell people that. Convince yourself of that.

Si tienes Cáncer de Próstata y necesitas Tratamiento Hormonal (castración médica), es posible que a veces te percibas a tí mismo como un Ángel, y es posible que te parezca o que temas que la gente con la que te encuentras y que conoce tu problema pueda percibirte de la misma manera.

¿Por qué un Ángel?, alguien puede preguntar: bueno, se dice que los ángeles no tienen deseo sexual, y tú tampoco lo tienes; además, mucha gente percibe a los ángeles como carentes de diferenciación sexual, y tú a veces te sientes igual: es posible que tus testículos se hayan reducido, es posible que tu pene haya casi desaparecido y tu vida sexual es simplemente historia.

Pero, de hecho, los Ángeles no son como tú en modo alguno. Dile eso a la gente. Convéncete a tí mismo.

Si, en cualquier caso, tuviéramos que usar metáforas, yo usaría la del ANGEL CAÍDO. Tiene el aspecto de un hombre, y parece como si todavía se estuviera preguntando el por qué....

What's up, Doc?

I reckon that only in our worst nightmares could we imagine ourselves in front of a doctor talking about OUR cancer.
Well, our worst nigthmares can become true sometimes and then reality turns out to be even harder than fiction. In the real world, is not just one doctor we have to be in front of, but in front of many potential ones, and we have to put our lives into their hands and have to choose the best pair hands to receive that precious gift.
Urologist: Your best choice, given your young age (47) and no clinical evidence of PCa spread is, is, and the winner is :Surgery; Radical Retropubic Prostatectomy. I am an expert surgeon, he added. You'll have not urinary incontinence problems, only a 50% chance of impotence and, as far as Cancer is concerned, only time will tell whether a cure has been achieved or not. If you find any doctor treating cancer who can promise you a cure from the beginning, then go to him at once....
Second opinion from a medical oncologist: Surgery? Man, what are you talking about! Your cancer has most likely spread already and surgery will not remove it all. Choose radiotherapy. If you choose surgery you will have to have radiotherapy afterwards in any case.
I made a decision. I had surgery. No incontinence, Ok the Doctor was right about the first guess. Impotence?: I didn't have enough time to check it out, because shortly after the surgery the pathological report came in and it proved that the medical oncologist was a better future teller: extracapsular extension, positive margins and one node out of ten positive for cancer from the lymphadenectomy.
Six weeks after surgery I decided to have adjuvant radiotherapy and adjuvant hormone therapy. Surgery+radiotherapy equals sexual potency vanished for ever. Add to this adjuvant hormone treatment and the magic mirror will say that you are perfect angel.

Me imagino que sólo en nuestras peores pesadillas nos imaginamos a nosotros mismos delante de un médico hablando de nuestro cancer.
Bien, pues nuestras peores pesadillas se cumplen a veces, y entonces la realidad es mucho más dura que la ficción. En el mundo real, no sólo es con un médico con el tenemos que tratar, sino con muchos médicos potenciales, y tenemos que poner nuestras vidas en sus manos y tenemos que escoger cúal es el mejor par de manos para recibir ese precioso regalo.
El Urólogo: su mejor elección, dada su edad (47) y dado que no existe evidencia clínica de que el CaP se haya extendido es, es, y el ganador es: cirugía; Prostatectomía radical retropúbica. Soy un cirujano experto, añadió. No tendrás problemas de incontinencia urinaria, sólo un 50% de probabilidad de impotencia, y sólo el tiempo nos dirá si se ha conseguido curar el cancer o no. Si encuentra a otro doctor que trate el cáncer que le pueda prometer una cura desde el principio, no lo dude, vaya a él.
Segunda opinión de un oncólogo médico: ¿Cirugía?. ¿De qué está hablando Vd.? Lo más probable es que su cancer ya se haya extendido y la cirugía no lo eliminará por completo. Escoja radioterapia. Si escoge cirugía, después tendrá que darse radioterapia de todos modos.
Tomé una decisión. Me sometí a cirugía. No tuve incontinencia, o sea que el doctor había acertado con su primera predicción. ¿Impotencia?: no tuve tiempo suficiente para comprobarlo, porque poco después de la cirugía llegó el informe de la anatomía patológica y demostró que el oncólogo médico era mejor prediciendo el futuro: extensión extracapsular, márgenes positivos y un ganglio de entre diez de la linfadenectomía contenía cáncer.
Seis semanas después de la cirugia, decidí someterme a radioterapia adjuvante, y a tramiento hormonal adjuvante. La cirugía + radioterapia era igual a una potencia sexual que se desvanecía para siempre. Añadamos a esto tratamiento hormonal adjuvante y el espejo mágico nos dirá que somos el angel perfecto.

Dr Gleason?, I presume...

To Say that one has Prostate Cancer is to say too little about that. In fact, many men die with PCa rather than from it, and many didn't even know they had PCa when they died and might have written a silent will for their descesdants.
Many men are diagnosed with PCa which is so little aggressive and it's found in so early stages that all they need is a close monitoring or what is called Watchful Waiting (WW)
Aggressiveness, the magic word. A PCa is said either not to be aggressive, a pussy cat, or to be as aggressive as a tiger depending on what Dr. Gleason, on the right side of the photo, has to say about our cancerous prostatic cells (on the left side, and holding rifles..). That's the Gleason grade or Gleason sum.
Another and very important fact to know it the stage or our PCa. Was it diagnosed early and thus may be curable or was it diagnosed at more advanced stages and a cure is unlikely?
Clinical (different to pathological) staging of our PCa is a critical issue for prognosis and treatment decision making.
When the meeting of the photo was over I got some responses: my biopsy gleason sum was 4+4 and my clinical stage was considered as T2a.
After surgery, I was told that there had been another crowded meeting and, this time ,the final agreement was reached on a pathological stage of T3bN1 and a Gleason sum of 4+4.
Thereafter, both groups dissolved and I was sent to the witch doctor's hut. Time to make an informed decision has arrived for a journey I would have never thought I had to take....

¿Dr. Gleason?, supongo

Decir que uno tiene Cáncer de Próstata es decir muy poco al respecto. De hecho, muchos hombres mueren con su CaP en vez de a causa de él, y muchos ni siquieran sabían que padecían CaP y puede que hayan dejado una herencia silenciosa a sus descendientes. Muchos hombres son diagnosticados de CaP que es tan poco agresivo y se encuentra en estadíos tan iniciales que todo lo que necesitan es ser vigilados estrechamente o, como también se llama, estar en una espera vigilante.
Agresividad, palabra mágica. Se dice que un CaP o bien no es agresivo, un gatito, o es tan agresivo como un tigre, dependiendo de lo que el Dr. Gleason (a la derecha de la foto) tenga que decir sobre nuestras células prostáticas cancerosas (al lado izquierdo y con rifles..). Es lo que se llama el grado de Gleason o la suma de Gleason.
Otro factor a conocer, y muy importante, es el estadío de nuestro CaP. ¿Fue diagnosticado a tiempo y, por tanto, puede ser curado, o se diagnosticó en un estadío más avanzado y es poco probable conseguir la curación?
El estadiaje (difente al patológico) clínico de nuestro CaP es un aspecto crítico para la prognosis y la toma de decisiones sobre el tratamiento.
Al terminar la reunión de la foto, obtuve algunas respuestas: la suma de gleason de my biopsia era 4+4 y se consideró que mi estadío clínico era T2a.
Tras la cirugía se me informó de que se había producido otra populosa reunión y, esta vez, se había alcanzado un acuerdo final sobre el estadío patológico: T3bN1, y la suma de Gleason:4+4.
A partir de entonces, ambos grupos se disolvieron y se me envió a la choza del curandero de la tribu. Había llegado el tiempo de tomar una decisión informada para un viaje que nunca pensé que tendría que hacer....

Fear of Diagnosis

Before I entered the doctor's office I knew already I had Prostate Cancer . Don't ask me why, but I knew. I had not symptoms at all but a yearly routine echography had detected something wrong.

While the doctor was putting a glove on his right hand and wiggling his fingers I could hear him saying: everything is going to be Ok, don't worry, you're too young to have Prostate Cancer (47 at that time).

When the finger reached his target and he suddenly stopped talking I realized how true my premonition was going to be.

He ordered a PSA test. It came back as 18.4 ng/ml. Then the biopsy: positive for Prostate Cancer. The ground seemed then to tumble down under my feet.

Why me? I kept on asking that for several days. Prostate Cancer? Impossible!, I'm just 47 and my diet has been healthy (my wife keen on "mediterranean diet) and no story of familiar PCa at young ages.

I began reading about PCa and the answer to my question came in easily: hundreds of thousands of men are diagnosed every year of PCa cancer around the world . Of these, about 15% are under the age of 50. It was me one of them, but could be YOU next time if you're reading this.

PSA test and DRE (Digital Rectal Examination) are a must if you're over 40 years of age if you want to have your potential PCa diagnosis be caught early and thus curable or at least manageable for many years.

I don't think mine is curable, but almost three years after my diagnosis I have enough strenght and I'm feeling well enough as to wish to share my "life with Prostate Cancer" with all of you.

The fear of diagnosis must be overcome by the wish to live.

El miedo al diagnóstico

Antes de entrar a la consulta del doctor, yo ya sabía que tenía cáncer de próstata. No pregunteis por qué, pero lo sabía. No tenía ningún síntoma, pero una ecografía anual de rutina había detectado que algo iba mal.

Mientras el doctor se ponía el guante en la mano derecha y movía sus dedos le odía decirme: no te preocupes, todo irá bien, eres demasiado joven para tener cáncer de próstata (47 años en esa época).

Cuando su dedo alcanzó el objetivo y él dejo de hablar de repente, me di cuenta de lo cierta que iba a ser mi premonición.

Me pidió un análisis de PSA. El resultado fue de 18,4. A continuación, la biopsia: positiva para cáncer de próstata. En ese momento, me pareció que el suelo me tragaba.

¿Por qué yo? Seguí haciéndome la misma pregunta durante varios días. ¿Cáncer de Próstata? ¡Imposible! Sólo tengo 47 años, mi dieta ha sido siempre saludable (mi mujer adora la "dieta mediterránea") y no tenía historia de cáncer de próstata en la familia a edades tempranas.

Comencé a leer sobre el cáncer de próstata, y la respuesta a mi pregunta fue fácil de encontrar: se diagnostica a cientos de miles de hombres de cáncer de próstata cada año en todo el mundo . De entre estos, aproximadamente un 15% tienen menos de 50 años. Yo había sido uno de ellos, pero si estás leyendo esto TÚ podrías ser el siguiente.

El análisis de PSA y el Tacto Rectal son una obligación si tienes más de cuarenta años y deseas que un diagnóstico potencial de cáncer de próstata sea cogido a tiempo y, por tanto, pueda ser curado, o al menos tratado durante muchos años.

No creo que el mío pueda curarse, pero después de casi tres años desde mi diagnóstico, estoy lo bastante fuerte y me siento lo suficientemente bien como para compartir mi "vida con cáncer de próstata" con todos vosotros.
El miedo al diagnóstico debe ser vencido por el deseo de vivir.

When life was better

I was 35. My parrot had no age or didn't confess it. Life was smooth. No children, my wife and I could travel freely to distant countries. Drugs, sex and rock&roll was our moto (don't take it literally).
Who was thinking of Prostate Cancer at that happy time? Not me, you can bet. Cancer of any kind could not touch me, could it?
Now, at 50 (see profile photo), I have a beloved son and Prostate Cancer....
But you know what: life goes on and, if not better, can be somehow good. This a blog about how to live with PCa and your views are welcome.

Tenía 35 años. Mi loro no tenía edad o no quería confesarla. La vida transcurría suavemente. No tenía hijos, my mujer y yo podíamos viajar libremente a lugares remotos. Sexo, drogas y rock and roll era nuestro lema (no lo tomeis al pie de la letra).
¿Quién pensaba en el Cáncer de Próstata en aquella época?. Yo no, desde luego. Ningún tipo de cáncer podía alcanzarme, ¿verdad?. Ahora, a los cincuenta (ver la foto del perfil), tengo un hijo al que adoro y Cáncer de Próstata...
